How do I change my address or name?

HQCB requires all address or name changes to be in writing. You can e-mail, mail or fax in the change information. We will need your complete name, previous name or address and your new name or address.

How do I find my CPHQ number?

The CPHQ number is located on both your certificate and wallet card. You can also call the HQCB executive office and we can look up your number for you. It is not necessary to have your CPHQ number when recertifying.

 

How do I find my certification expiration date?

The certification expiration date is located on your CPHQ wallet card. You can also call the HQCB executive office and we can look it up for you.

Recertification

Do you notify me when my certification is due to expire?

The HQCB executive office mails out a recertification applications six months prior to your expiration date and a reminder postcard and e-mail is sent to your last known address three months prior to your expiration date. It is necessary for you to notify the office of any address changes. Ultimately, you are responsible for your recertification and your expiration date.

What is the fee for recertification?

The recertification fee is $175.00.

What does HQCB accept for continuing education?

For individual recertification HQCB accepts any continuing education that follows the content outline for the exam. The content outline is available in both the candidate handbook and the recertification handbook. One hour of class time equals one CE hour. Please note courses relating to clinical patient care or medical treatment are not accepted. All other guidelines for recertification are also available in the recertification handbook.

Can I send in my continuing education prior to recertifying for review?

HQCB does not review courses on an individual basis for CEU’s unless the provider of the program has requested the program be reviewed for HQCB hours by filling out the CEC Approval Form and paying the appropriate fees.

What happens if I don’t recertify by my expiration date?

You have a one month grace period after your certification expires to submit your recertification application. It must be postmarked by January 31 of the year following your expiration date. If you fail to submit your recertification by that date, you will be required to sit for the examination to become CPHQ certified again.

What are the fees to sit for the CPHQ examination?

NAHQ member fee: $370.00
Non-member fee: $440.00

What are the eligibility requirements for the CPHQ examination?

In 2003, the HQCB Board of Directors voted to eliminate the minimum education and experience requirement to take the CPHQ examination. However each candidate must take time to assess and judge his/her own readiness to apply to take the CPHQ examination particularly if you have not worked in the field for at least two years. A careful review of the examination content outline and the sample questions provided in the candidate handbook is advised.

How long do I have to take the examination once I receive eligibility confirmation?

You have 90 days to schedule and sit for the examination once your application has been approved.

Yes. You can apply for the examination and schedule your examination on-line. We have a link to Applied Measurement Professionals, the testing organization, on the HQCB website or you can go directly to the testing organizations website at www.goamp.com click on Candidates and follow the instructions.
